The Community Health and Social Services Network (CHSSN) is seeking a bilingual Patient Navigator — that person will provide guidance with appointments and forms, accompany patients to visits, help with lodging and transportation, and ensure their needs are understood by health providers. About the Role Reporting to the Project Coordinator – Patient Navigator, this position involves supporting English-speaking residents of Eastern Quebec traveling to Quebec City for specialized medical care. This role provides logistical, emotional, and informational support to patients and families, ensuring equitable access to health services. Key Responsibilities Program Delivery Navigation and Guidance: Assist patients with appointment logistics, medical forms, and understanding health information. Liaison and Accompaniment: Act as a liaison between English-speaking patients and health providers; accompany patients to appointments and provide emotional support to them and their families. Identify and Address Barriers: Help patients overcome challenges such as transportation, lodging, language, and cultural differences. Advocacy and Collaboration: Advocate for patients’ needs and collaborate with health professionals to ensure appropriate care. Monitoring and Reporting Track patient interactions using data collection tools. Provide regular reports to the Project Coordinator. What You Bring Bilingual (French and English), with strong communication, problem-solving, and leadership abilities. Ability to work collaboratively with diverse community partners and stakeholders. Strong organizational and project management skills, with attention to detail and deadlines. Empathy, cultural sensitivity, and creativity in problem-solving. Proficiency with digital tools for communication, reporting, and collaboration. Flexibility to occasionally work evenings and weekends. Qualifications Bachelor’s degree in Health Sciences, Social Work, Nursing, Psychology, or Community/Public Health preferred; college diploma with relevant experience considered. 2–3 years in health care, social services, or community support (3–5 years preferred). Knowledge of Quebec City’s health system, local hospitals and landscape, an asset. Bilingual (French & English), strong communication and problem-solving skills. Driver’s licence to be able to drive in Quebec. Work Environment Location: Office-based in Quebec City, with travel across Quebec as needed. Flexibility for occasional evening/weekend work.
