With all 12 Patient Navigators now fully hired, trained, and actively working within community-based partner organizations, the network has already provided tailored, compassionate support to hundreds of English-speaking patients and families navigating Quebec’s health and social services system. Launched by CHSSN in April 2026 with $995,000 in funding support from the Secrétariat aux relations avec les Québécois d’expression anglaise (SRQEA), the Patient Navigation Network was created to help address linguistic and systemic barriers to care. Through close collaboration between CHSSN and its regional partner organizations, what began as a targeted provincial rollout is now fully deployed across 12 key regions in Quebec. Empowering Patients Across Quebec Navigating healthcare can be complex and overwhelming, but language barriers present significant additional challenges for English-speaking Quebecers. Clear communication during consultations, treatments, and follow-ups is critical to patient safety, comfort, and positive health outcomes. As an initiative of CHSSN, the Patient Navigation Network provides a free, confidential service that bridges this gap. Patient Navigators work directly with individuals and families to: Provide Personalized Guidance & Accompaniment: Offer direct assistance, accompany patients to appointments, and ensure effective communication with healthcare providers. Overcome Systemic Barriers: Help complete medical forms, clarify complex instructions, and guide patients through administrative processes. Coordinate Care & Logistics: Connect individuals with local health and social resources, arrange transportation or lodging support, and coordinate referrals for specialized care outside their home regions. A Strong Network Across 12 Regions The 12 Patient Navigators are housed within trusted local community organizations across 12 regions: Est-de-l’Ile-de-Montréal: East Island Network for English-Language Services (REISA) Estrie (Eastern Townships): Townshippers’ Association Lanaudière: English Community Organization of Lanaudière (ECOL) Laurentians: 4Korners Lower Saint-Lawrence (Rimouski): Heritage Lower Saint Lawrence Montérégie Centre: Assistance and Referral Centre (ARC) Montérégie East: Montérégie East Partnership for the English-Speaking Community (MEPEC) Montérégie West: Montérégie West Community Network (MWCN) North Shore (Sept-Îles and Baie-Comeau): North Shore Community Association (NSCA) Outaouais: Connexions Resource Centre Quebec City: Jeffery Hale Community Partners Saguenay–Lac-Saint-Jean: English Community Organization Saguenay–Lac-Saint-Jean (ECO-02) The Network also includes 2 CHSSN-affiliated Patient Navigators from the Youth & Parents AGAPE Association Inc. (AGAPE) in Laval, and the Committee for Anglophone Social Action (CASA) in Gaspésie. Proven Impact and Growing Reach Since launching earlier this year, CHSSN’s PNN initiative has assisted hundreds of clients across urban, rural, and remote areas of the province. With all 12 Navigators now actively serving their local communities, and a centralized website, the network is well-positioned to expand its reach so that no English-speaking resident has to navigate care alone. “Reaching full operational capacity with all 12 Patient Navigators across Quebec is a landmark achievement for CHSSN and the communities we serve,” said Jennifer Johnson, Executive Director of CHSSN. “With the generous support of the SRQEA and the dedication of our regional partner organizations, we are reinforcing vital links between English-speaking Quebecers and the healthcare system.” “Having worked on the ground as a Patient Navigator in Quebec City for eight years before taking on the role of Program Coordinator, I know firsthand how transformative this support is for patients and their families, as well as healthcare professionals” added Steve Guimond, PNN Project Coordinator at CHSSN. “Seeing this network grow to 12 Navigators across 12 regions is in all honesty a dream come true.
